For the domain issue with self signed certs, i've been having luck with a domain name registered that i use only for internal services behind a caddy reverse proxy using porkbun api for verification and pihole to help route the internal traffic.

To the comment by Patric Jacobson – yes you can use a VIP for outgoing traffic. You have to create a NAT rule via Firewall – NAT – Outbiund
What I don't like about Immich is it doesn't seem to be made to point at an existing folder of photos and be a front end for that the way Jellyfin works. It's built around uploading photos directly to it.
